PRIVATE PAY
The cost per session is $125.00. All therapy sessions are 50 minutes long and are conducted online through a secure client portal. Clients can pay by Credit Card, Debit Card or Health Savings Account card using Linville Counseling Services’ Square account.

*A referral from your primary care provider (PCP) is required for these Anthem IN Medicaid plans. You can give your PCP our Business Fax number: 317-830-0123.
You can request an appointment with me here. Please write a note in the request that you have Anthem IN insurance and I will send you an insurance form through the encrypted therapy portal. I will check eligibility and contact you before our first appointment.
Optum from United Healthcare
I accept Optum insurance (as of February 1, 2025).

Please mention that you have Optum insurance in the appointment request so I will know to send you an insurance form so I can run an Eligibility & Benefits report to find out what your session fee will be. I will contact you before the first session so you can decide if paying your insurance’s negotiated rate is ok with you.
Other Insurance
I am not an in-network provider with any other insurance companies.
However, I do offer out-of-network benefits by providing you with a superbill after you pay for your session upfront.
A superbill is a document containing information about your therapist, your diagnosis and the procedure code that insurance companies require to give you partial reimbursement if the therapist is not in the insurance network. You will have to check with your insurance company to see if they offer this out-of-network benefit. Additionally, some insurance companies will take the superbill and apply it towards your yearly deductible.
